Fraxel Re:store

In our practice, the Fraxel Re:store is used for photo rejuvenation, mild sun damage, mild wrinkles, acne scars, surgical scars, melasma, stretch marks, enlarged pores and hypopigmentation.  We are also seeing tightening of the upper and lower eyelids and neck.  The need for repetitive treatments is variable depending upon the condition. 

The Fraxel Re:store utilizes the concept of fractional photothermolysis.  This concept allows for the fractional treatment of pixel-like zones of thermal damage that does not create wounding or significant side effects yet allowing for therapeutic benefit.  The first laser to utilize the concept of fractional thermolysis was the Reliant Fraxel laser.  It is a mid infrared laser that creates a variable density of 100 to 160 micron wide columns of thermal damage known as microthermal zones.  These microthermal zones penetrate from 300 micron to 1.3 mm into the dermis depending upon the energy level used.  This pixilated fashion in which the skin is treated leaves a majority of the skin undamaged to facilitate rapid repair.  Patients typically see three to five treatment sessions spaced 21 to 28 days apart.

Preparation for the procedure necessitates the application of a topical anesthetic and air cooling.

Side effects from fractional resurfacing are limited and usually include redness for several days and mild to moderate edema depending upon the location of the treatment.  Post-inflammatory hyperpigmentation can sometimes be seen as a short term side effect; however, this is not seen over the long term.  In contradistinction to older resurfacing procedures, hypopigmentation is not seen.

A distinct average of fractional resurfacing compared to ablative techniques is the ability to treat off the face including the neck, chest, arms and hands.
