This top surgery removes breast tissue to masculinize the chest contour for transgender men and nonbinary people. If there is not much breast tissue to remove, the nipples and areola may be left in place during the procedure. Other times, we will need to remove the nipples and areola and then place them back on the chest in the correct position. While this approach does require more incisions and results in loss of nipple sensation, it may produce the most natural result.

Some patients may require more than one surgery to achieve the result they want. Patients with risk factors for breast cancer may decide to have nipples and areolas removed and tattooed on after the initial procedure. Recovery from top surgery usually takes several weeks.

Transgender women and nonbinary patients may use hormone therapy to stimulate breast growth, but this process often doesn’t give them the size they want. Implants are placed in the breast tissue to create fuller, more shapely breasts. If the breasts are very small, even after hormone therapy, we may recommend expanders in the tissue first to stretch the skin slowly and allow for the larger implants. Recovery from implant surgery typically takes a few weeks.

Fat grafting is another option for patients who want a more modest increase in breast size. This procedure removes fat from one area of the body via liposuction and injects it into the breast tissue. When the fat is properly removed and harvested, the results of fat grafting can be long-lasting. Downtime after this procedure is typically just a few days.

In addition to a physical examination, surgeons and insurance companies usually require people seeking gender-affirming surgery to provide a letter of support from a mental health provider competent in transgender health. This provider will determine whether you meet the standard of care for the surgery you want, based on the following criteria:

• Persistent, well-documented gender dysphoria
• Ability to make a fully informed decision and to consent to treatment
• Legal age to make health care decisions (18 years old in US)
• Effective management of significant medical or mental health concerns
